When clinically appropriate, our clinicians may assist with accommodation request, support letters, and other mental health-related documentation as part of ongoing care.

Documentation Policies & Requirements

To ensure accurate and ethical documentation, requests for forms and letters are reviewed according to the following requirements and policies:

Requirements:

  • Must be an established client

  • Typically 3-6 completed sessions

  • Actively engaged in treatment

  • Documentation is based on clincial judgment and is not guaranteed

Turnaround Times:

  • Standard 5-10 business days

  • Expedited options available for an additional fee

Important Note:

Additional appointments may be required. Documentation fees are separate from therapy session fees. Completion of forms, letters, and accommodation requests is not guaranteed. All documentation is based on clinical appropriateness, treatment history, professional judgment, and ethical guidelines. Additional appointments and fees may apply.
